A compassionate and trained care manager will be available to all Sages Elder Care clients and families free of charge. The care manager will be the first person you meet to explore all services provided by Sages Elder Care and to create a personalized care plan with your needs in mind. The care manager will be your go to person to answer questions related to cost of services, policy and procedures, and client/staff safety.Your care manager will help the client and client families stay informed through our online and print resources. Your care manager will check in with each client monthly to gain customer feedback and to assess/inquire about changes in level of care. Care managers will also check in with each client and the client emergency contact when inclement weather threatens the safety of our clients. Client safety and the clients right to self determination will always be a priority as our staff are considered to be guest in your home.
Monthly charges are determined after a free consultation with a care manager. A care manager will come to your home to explain services available and cost associated. A personalized care plan is then developed. After the care manager and client have agreed on care plan, Sages Elder Care LLC will present the cost of services provided, explain the monthly billing process and shared client rights information prior to services being offered in the home.
